Creating a PO in Request Resolution

When clicking the OK button to create a PO in Request Resolution I am getting an error message that says...

Unhandled script exception:
Object has no reference.

EXCEPTION_CLASS_SCRIPT_BAD_PARAM
SCRIPT_CMD_DEREF_TABLE

And PO will not create...
